Passports
Westminster City Hall (4800 W. 92nd Avenue) provides passport services for:
- New passport applications (DS-11) for adults and children
- Renewals for children
- Adult renewals (DS-82) are handled at Westminster City Hall under certain circumstances. Adult renewals may typically be done online or by mail, without the assistance of a passport acceptance agent. Please visit the U.S. Department of State's website for more information.
- Appointments are required.
- Please call 303-658-2400 or email Passports to make an appointment. Please include a phone number in your email for quicker scheduling.
- Appointments are also available with a Spanish-speaking passport agent
Passport applications must be completed prior to your appointment. Passport forms are available at the City of Westminster City Hall or online at the U.S. Department of State's website.
Prepare for Your Appointment
When you appear for your appointment, you must have the following:
- Completed DS-11 application (black ink only). Do not sign until oath is given by agent.
- Please print single sided, do not print on both sides.
- Proof of U.S. citizenship
- State-issued birth certificate filed within one year of birth, Naturalization certificate, or expired adult-issued U.S. Passport
- Photocopies are not accepted
- One passport photo
- Photos can be obtained at a self-service kiosk in the City Hall lobby during business hours for $15, credit/debit card only.
- Walgreens, FedEx and the UPS Store also offer passport photos; please contact these companies directly for more information.
- State-issued photo ID (for those 16 and older)
- Copy of state-issued photo ID (both front and back on one side of paper)
All Applicants Must Be Present Including Children
- Children 15 and under must have both parents present with ID. If one parent cannot be present, Form DS-3053 (PDF) needs to be filled out by the non-appearing parent, notarized, and accompanied by a photocopy of the front and back of that parent's ID. - Children ages 16 and 17 need one parent present with ID
Payment
- Acceptable forms of payment include money orders, personal checks. No starter checks or temporary.
- No cash or credit cards.
- Fees are split in two - one payment for the State Department, one payment for the City of Westminster

- Adults (16 and older): two checks
- U.S. Department of State: $130, City of Westminster: $35
- Child (15 and under): two checks
- U.S. Department of State: $100, City of Westminster: $35
- Adults (16 and older): two checks

Current processing times fluctuate. Passport applications may be expedited for an additional fee. If you require your passport within two weeks, visit the U.S. Department of State's website.
Passport Expiration Dates
- The U.S. Department of State recommends your passport have at least six months of validity beyond your actual travel date to avoid expensive and lengthy travel disruptions. The normal rule for most countries is that they require you to have three months from your actual travel date before your passport expires.
- U.S. citizens traveling on passports that expire in fewer than six months have increasingly been denied airline boarding or have been detained upon arrival in certain foreign destinations, including popular European travel destinations in the Schengen area.
- This is not a new requirement, but it is only recently that the requirement has been more strictly enforced.
